Cody Allen Upchurch, age 13, Puryear, TN, was DOA at Henry County Medical Center ER on Saturday, June 21, 2008. His funeral will be conducted at 10:00 AM Wednesday at McEvoy Funeral Home Chapel. Burial will be in Crockett Chapel Cemetery in Lake County, TN. Visitation will be from 4:00 - 8:00 PM Tuesday at McEvoy Funeral Home and after 1:00 PM Wednesday at the gravesite in Lake County.He is survived by his father, Thomas J. (Peggy) Upchurch, Puryear, TN, mother, Lisa Kiddy (John "Buddy") Bates, Ripley, TN, a sister, Ashley Bates,paternal grandparents, Phillip and Christine Upchurch, Gleason, TN, six aunts, one uncle and a number of cousins. His Maternal Grandparents, Henry C. and Margie Kiddy are both preceded.
