Survived by two sisters: Jackie Mihalik, Paris, JoAnn (Larry) Lewis, Mesa, AZ; brother: Phillip Cotham, Paris; numerous nieces, nephews, great nieces and great nephews. Jerry previously worked at Plumley Companies and was in the U.S. Army.
